Not to fear, Blanchard and Johnson address these concerns in their short simple book. The allegory starts off with a young man in search of an effective manager. Initially disillusioned by the managers he encounters, who are only results-oriented at the expense of the employees or only people-oriented at the expense of the organization, the young man discovers The One Minute Manager. The young man learns from The One Minute Manager and the people whom he manages the philosophy of the one-minute management style. The authors gradually convince the readers through examples, anecdotes, explanations, and quotable quotes why and how their three principles, when followed appropriately, actually work. A brief guideline list accompanies each of the three management skills: the "one minute goal setting," "one minute praising," and "one minute reprimand." There is even a concise flow chart to help solidify the management principles into one page near the end of the book; no doubt, designed to be cut-out or photocopied and posted in every manager's office. By the end of the short 60 minutes required to finish the book, most readers will be convinced to at least test out the method. After all, the title of the book already suggests that time commitment is not an issue; one can become an effective manager by investing a short 60-second of time. When one reads the book, this really translates to several 60-second intervals per day, but the amount of time will be much less than what most managers are used to. More effective management in significantly less time? Is it possible? The authors certainly make a compelling yet uncomplicated and clear case for this. One does have to wonder if this is too easy. Will it work for managing all types of people in all types of organizations? How about the slackers? Or the employee who is the wrong fit for the job, doesn't have the appropriate training, or lacks motivation? How about the company with a long tradition of top-down management style? The book doesn't get mired in the details or specifics of every possible variation or situation; instead it attempts to provide the general strategies of effective management. The simple message of the book is to focus on making people feel valued which will lead to greater self-motivation and increased productivity for the individual and organization. As the One Minute Manager best described it, "People who feel good about themselves produce good results." And because they are not difficult to understand, learn, or implement, the skills can be readily used by most managers immediately. The book is not only for managers of people. The message from the book is relevant to anybody interacting with people or when managing your own work and life. For example, I can utilize the skills to not only manage the people who report to me, but also in working with my colleagues or my supervisor, although the lesson on reprimanding may be less applicable in such situations. The most useful setting to apply the skills, perhaps, is when teamwork is involved, such as in committees, meetings, and group projects. This is when setting goals and timelines are critical and working effectively with colleagues, including giving praise and constructive criticism, is paramount to success. Furthermore, I can even see me being a One Minute Manager to myself, be it in the work environment, personal life, educational pursuits, or participation in hobbies. Who wouldn't want to be happier and have better results in all of these activities? I wouldn't mind impressing my boss more, fulfilling all my New Year's resolutions, or running a marathon in 3 hours and 15 minutes. After reading "The One Minute Manger", most readers will want to be a One Minute Manager tomorrow.

This reader studied Cleverly's text as a requirement for a healthcare finance class. The text was frustrating as this reader attempted to "know" everything about healthcare finances; however, this is probably not the intent of the book. It does present a road into the foreign land of finances. It demands respect for another set of data and another language for interpreting that data. One does not master this data set at one pass, however. Nevertheless, this reader did gain some new financial information. At the risk of being simplistic, but communicative; a listing of some of the concepts learned follows: 1. There are many users of financial information. 2. Financial information can guide the formation of programs. 3. Financial management is essential for successful healthcare organizations. 4. Various qualities of health care organization types. 5. How health care organizations make up for discouting and bad debt and capitated payments. 6. General principles of accounting and why they are important. Also that these principles still need to be explained, to be consistent, and to be clarified. 7. Overview of four main types of financial statements with a brief explanation of vaious line items. 8. That financial planning includes considering inflation before it happens, and for equipment etc. to break and wear out before it breaks or wears out. 9. That financial information can be better understood by comparing financial ratios of different line items and trends over time. There are national benchmarking ratios available and Cleverly gives some and tells how to get more. 10. That financial planning should be an orderly process in an organization. 11. There are different types of costs. Some stay the same, some are overhead types. Some are direct, some are hidden. Some can be controlled, some cannot. 12. Figuring out prices is a very complex process in healthcare. It is based on costs and payers. It must also include indirect costs and costs of future problems. 13. There is still stuff to learn . . . And this reader is still no expert. Finally, after this MSN course and this text, this reader is more conversant with the financial landscape. It holds interest and it is understandable, after all.

The format of the book is to present evidence for quality problems in healthcare in America and make recommendations. The operational definition of quality used in the book is "The degree to which health care services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge." There are thirteen recommendations presented initially and are discussed in relevant chapters. The recommendations vary in scope from suggesting that multiple parties need to be committed to quality as a way to decrease the burden of disease to suggestions that specific agencies fund pilot studies to look at how reimbursement can be aligned with quality. Six major parameters are discussed as guiding quality and it is suggested that 15 specific conditions be a focus for improving quality. There is no difficulty in identifying literature studies that demonstrate quality problems in hospital and clinical populations. A survey of current research is included in Appendix A. A review of the tables in this appendix show the types of quality markers that are typically studied in the literature. The authors make the argument that errors due to quality lapses or deficiencies need to be actively worked on and that the current high error rates are not acceptable. Health care has become a major political issue and the political factions are shaping up to be government and business on one side and physicians and other health care providers on the other. There has been a major revamping of the health care system in the past decade to control costs. That required the active cooperation of the insurance industry and government. There is still medical inflation and limited access with 40 million Americans uninsured. Should we believe that another cooperative effort between industry and government will improve quality any more than it has controlled cost or improved access? The authors acknowledge weaknesses in their suggestions about changing the face of American medicine, but they minimize the adverse impact of the current funding mechanisms for medical care and the issue of information systems integration and security. A good example can be found in their application of engineering principles to clinical settings - - where teams see patients for four hours of direct contact time and the remaining time is for documentation and returning calls. That plan would not be economically feasible in many settings. The high cost and lack of flexibility of the current reimbursement schemes are not mentioned as a potential reason why these plans won't work. Information technology is seen as a way to enhance both productivity and safety. The authors suggest that e-mail can lead to productive exchanges between physicians and patients. Many physicians have been doing this for years. Many have also stopped with the advent of security concerns about medical privacy. With larger IT systems the critical issue is backward compatability with older systems. That usually requires custom designs that are extremely expensive. Those problems usually need to be solved before bedside computing and decision support can be developed. Security is acknowledged as a problem that needs to be solved. In spite of a federal initiative in this area, the important precedent to remember is how the financial privacy of Americans was protected. The authors point out that medical privacy requirements need to be more stringent than other industries. At the same time they point out that some opinions suggest that there is a trade off between privacy protections and the need to advance information technology in health care. If they are suggesting that the Internet should be at the heart of this infrastructure and the Internet is not secure, what does that mean? A practical approach might be to focus on the areas where data is entered into computer systems and make sure that non-human analysis occurs at those levels. For example, all hospitals enter pharmacy orders into computer systems. Many hospitals require that physicians write separate discharge orders. Both of these points are areas where there could be immediate improvements in accuracy. A focused study and solution could be engineered now. The necessary software and hardware requirements could be placed on a central web site and available for download by hospital and clinic IT staff. Existing reviewers could be charged with documenting the baseline level of errors and the degree of improvement. This book succeeds as a broad survey of what has been done about quality in certain settings. It contains some interesting ideas about what can possibly be accomplished by applying conceptual advances from other fields. It does not discuss the significant drawbacks of evidence based medicine. It lacks a practical plan for transitioning to a new system and in effect creates a new chasm. With a work like this, whether you like the conclusions depends a lot on your interpretation of the evidence and your personal experience. As a practicing physician and a previous quality reviewer I have significant areas of disagreement with what is presented in this book. Areas of controversy are not elaborated upon. I suppose you could say that level of analysis is not required, but recommendations about the future of health care in America should at least meet the criteria of "evidence based" and all the evidence should be discussed.
