Palliative Care in the Home is a gem of a book, written by two noted palliative care specialists who were formerly general practitioners. First and foremost, Doyle and Jeffrey are concerned with quality of care. This reviewer believes their book will be of as much interest to nurses working in the home as to the physicians.

The book addresses special problems and challenges faced by both doctors and nurses in delivering palliative care within the home setting. There is special focus on home nursing issues. The authors maintain that good palliative care ought to begin in the home whether or not the patient is eventually admitted to hospital or special palliative care unit.

This small reference book is written in a straightforward style, and so is very readable and understandable, for the medical professional as well as the general reader with some knowledge and interest in the field of palliative care. The importance of the multi-disciplinary team is stressed throughout, along with the ethical need to respect patient autonomy and decision-making. Patients and families must be encouraged to express their feelings and views, and so take an active partnership role in providing care. Needs of the family carers and stresses on palliative care providers are among other issues developed.

The reader will find extensive, current material on symptom diagnosis, appropriate palliation and guidelines for medications to manage pain for a wide variety of diseases. This book is a must for caregivers and professionals facing the challenges of Alzheimer's. It is the first book I've read devoted to Alzheimer's care that offers something more than reasons to institutionalize our people. Caregivers need information, options and where to access tools and resources before they can make informed decisions about their choices or their role as caregiver. As someone who spent 25 years designing rehabilitations plans for handicapped people and another 10 years facing the care of my husband, Tom, who died of this disease in our home, this is the first volume I've picked up that speaks of caregiving in positive, life-giving, do-able terms and not the drudgery that we are all led to believe consumes the process. While it offers pragmatic, workable, affordable solutions to caregivers, it never loses sight of the continued humanity and needs of our family members for love and regard in spite of their loss of cognitive skills. My experience as a caregiver was met with nothing but obsticles in my quest to keep my husband home, a journey I relate in my book, "He Used to be Somebody." I found Tom's care problems were 75% attitude and 25% tools, resources and help. This book offers both to families. I highly recommend it.

Mark Warner's book on Alzheimer's proofing your home is more than a how to book. Mr. Warner gives an in-depth description of the ways in which Alzheimer's disease affects a person's life, and the life of the extended family. By understanding the disease in depth and the changes that take place in a person physical and emotional need, there is a context to understanding why modifications and alterations are needed. He explains what to do and why to do it. His approach is that of a loving caregiver who not only wants to make the home safe and secure, but also wants to raise the quality of life. . Modifications are practical, but also designed to bring comfort. Even after over two decades of working professionally with people with memory loss, many of his suggestions caught me by surprise. I kept saying to myself, "why didn't I think of that?" He addresses all the behaviors associated with the disease, from wandering to agitation, and suggests environment solutions that could ease the situation. Not only will this bring a higher level of comfort to the person with the disease, but many of the suggestions are a benefit for anyone who wishes to have a safe and barrier free home. I recommend this book to anyone caring for someone with Alzheimer's, anyone involved in home care, and anyone who is interested in altering environments to meet specific needs of unique individuals. Like any thoughtful book, his ideas about adapting environments to the needs of people with Alzheimer's disease also stretches one's thinking about changing environments for anyone with specific needs, or who wants to think about the highest level of safety for family members.

"...Alzheimer's Proofing Your Home" is the most complete, most helpful, and most insightful volume I have read on the subject of AD care. With incredible attention to detail, every issue one could encounter is addressed individually. It isn't enough to have a how-to-do-it manual with AD - Mark confronts matters like behavior, wandering, mobility, safety and comfort with deep sensitivity and compassion. A rich, in-depth source of information, Mark explains in detail how to transform any home into a safe haven in a manner that the patient will find warm and welcoming. Cognitive, spatial, and emotional deficits can be compensated for using Mark's well thought out guidelines, and I wish his book had been around when I began caregiving many years ago. There are many books on the subject, all filling a need; but Mark's is far and away superior to anything I've read, which is considerable. Hundreds of situations and their remedies are explored. Concerns encountered as AD progresses are addressed one by one, with a wealth of hints, tips, and shortcuts that are the lifesaver of harried caregivers. Easy to accomplish safety and comfort measures are provided - how needed! Emphasis is placed on concerns all CGs share - how to keep loved ones protected and content, AD being the antithesis of both. Mark has given us an instructional book, made warm and personal because it's about individuals and their needs. There is a comprehensive listing of product and service providers for caregivers, and most of the numbers are toll free. The title of this book does not begin to do it justice, it's so much more. I have a relative with AD. We are building and apt for him and his wife. This book has been the greatest resource for the contactor and care giver. Not only does it address medical problems but give suggested solutions and where to get the resource. If one were attempting to redue an existing apt/ house it is very easy to use. My local code enforcement officer has been amazed at the way problems can be adressed and still meet codes. Contractors get to understand more about the needs and how to plan ahead. I received "Home Rehabilitation: Guide to Clinical Practice" By Anemaet & Moffa-Trotter 2000 and LOVE it!
I think it is especially good for those new to home care.
It has sections on setting schedules, equipment, infection control, safety, regulations, medications, ethics, and pediatric and adult conditions. There is an extensive listing of evaluations, which are described in usable detail.
Common medications are listed by both generic and brand names, making them easy to track down, and side-effects are also listed.

My brother and I take care of my eighty-year-old mother. She has
diabetic neuropathy and can't walk. But she's a good kid and
we take care of her. This book has helped. Although the object of the book at first seems to be to develop a care plan; it really is a book of primary skills that you need in caregiving.
The book is best because you are not overwhelmed with 465 ways
to help your loved one. This is a primary and basic skills book.
Buy this book if you are new at caregiving and don't know where to start. Because the American Medical Associarion wrote it, I imagine that it would be good for all caregivers to buy it. I just love this book. It's like a Boy Scout manual. It's user-friendly for your brain. And sometimes simplicity can do more good that needless complications and theories. "Your book is a treasure. I wish I had read it 5 years ago. It definitely would have made my journey easier!
Appendix 2, Accessing care in Canada, and Appendix 4, the Questionnaire for viewing Nursing Homes, are invaluable. It took me months to figure those facts out for myself ... particularly the route to accessing care facilities, and the awareness that with costs based on income, they can be affordable to anyone. Most people assume that costs are prohibitive except for the wealthy, and for that reason they do not consider "care" an option for themselves. How can we make people aware of the many excellent, affordable facilities around? Public awareness, especially of Intermediate Care facilities, is important as so many seniors struggle independently, endangering their health and safety, unaware that Intermediate Care would indeed be not only suitable but also affordable for them. A number of doctors, to my knowledge, are even unaware of how the system works!
Chapter 17, Difficult decisions, you presented very factually yet
compassionately. It is so important to talk about the death of a loved one both before and after the event. Fear can be reduced and grief can be acknowledged. The care histories are so relevant. Hearing about others' anger, guilt and need for balance in personal life "hits home" with anyone who has had to put a loved one in care."

I have been working in the field of elder care for most of my adult life. I also have aging parents, and confront this issue in my personal life. Silin's new book is compassionate and senstive, and offers a complete view of the nursing home process.

It is unique in that it offers much pragmatic information, but also helps the reader confront emotional issues. One way this is achieved is by the use of vignettes written by caregivers. These stories allow the reader to read other family member's perspective, which gives added insight to the nursing home industry.

Silin also writes at length about such emotionally charged issues as abuse, guilt, loss and grief. He doesn't just raise these issues, but gives guidance and advice. This certainly will help people to deal with these painful issues.

Although he is honest and direct, he never under estimates the value of good and caring people who work in nursing homes.

After reading this book book, you will have a much more complete understanding of the problems both of nursing homes, of the people who live in them, and of their family and friends.
