I am an aspiring doctor and this text was a required reading for a pediatric internship. I read this book before my internship began and it helped me to get through this heart wrenching experience. At first it was difficult to read just because it really tugs at your emotions. However, once I made it past the first few stories I could not put the book down until I finished. Myra Bluebond-Langner does an incredible job of not only telling the stories of dying children, but she also gets into the innocent minds of the children enduring this difficult battle. This book is a must read for any aspiring pediatrician, or any aspiring physician for that matter. The way an innocent child views the innevitable can shed some light on how others may view the same situation. Althought parents might find this a tough read, it may help them to understand their child's point of view as well.

I work with issues involving the understanding of children in similar situations. What bluebound-langener has done is more than just tell us what goes on in the mind of sick child. She moves us to understand finally, that children do not fear entirely for their own lives, but vigilantly stand proud in the faces of death, subtly comforting thier doctors through thier smiles while at the same time watching their parents watch them die. 5-0 out of 5 stars My daughter is feeling and saying things never said before!!
I read this book thinking it "sounded" good but how could I use the info its seems quite complex and overwhelming. THen I attended one of Dr G's conferences and realized the book was never meant to stand alone!!! It is needed to progress and have a reference for the stages as you do them but RDI requires coaching and support. Ill tell you from me and many families who have done it, it has been the one thing that finally tackles the issues no other methodology can!!! We have been doing it for merely 2 months with an agency certifying throught DR G's practice and we have experienced spine tingling miracles. Most importantly it has made my daughter EASY to take ANYWHERE better than any ABA or VB has. In all honesty we have used many different methods, PRT, ABA, DIR-Floortime, Incidental Learning, You name it weve been there. But the commonality is that they still all allow the child to stay in their comfort zone- they are basically child centered approaches. This method MAKES your child come out of their shell, forces them through mutual enjoyment and love for the interaction to allow themselves to be lead.
The residual of all of the other methods was that they left my child having a hard time KEEPING friends due to her need to control situations (PRT and flootime) or be depended on adults to always allow for things to happen (ABA). THis changes all of it!! Not only is she beginning to appreciate and read social cues in just the mere first level of this method, but she is asking more age appropriate why questions and asking about things that happened to her when she was NONVERBAL!!! Her school is on board and wanting to do this and we know we cant stop. 2-0 out of 5 stars Esoteric tone overwhelms the practical suggestions
My overall impression of this book is that is contrived and forced in places. The book is structured around a layered model of progress; essentially progressing the student from the lowest level of competence on through higher levels. One might presume that the authors were stuck for a teaching metaphor and grabbed the apprentice-expert metaphor as scaffolding on which to hang the lessons. The book is replete with various lesson plans and tactics for enagaging the AS person's interest. To be fair, a reasonable amount of what is here is worthwhile reading and reflecting on - once you filter out the esotericism of much of the language.

However, the book seems to stumble as it tries to fill its 400 odd pages with 'advanced' lesson plans. Some of the section titles made me wonder was this a case of Asperger Syndrome meets Zen and the Art of Motorcycle Maintenance. The AS subject moves through Novice, Apprentice, Challenger, Voyager and Partner levels as just one example. There are sublevels within each of these. Finding one's unique idenitity and place in the environment etc, are major objectives.

The tenor of a lot of this, for me at least, verged on almost cultish twaddle. I prefer a reasonable scientific tenor to any behavioural modification programme. However, others may find the book's approach illuminating and helpful.

The book emphasises coaching and that the real business of interventions is coaching. My problem with this is that it doesn't tie coaching into any particular theory - though if you take one of the authors' RDI courses presumably the theory will be revealed. There is a growing emphasis on putting intervention programmes on some sort of scientific footing, and it behooves the authors of such programmes to produce the goods on the worth of their offerings.

The latter sections of the book, in my opinion, presume a lot of the AS subject. In particular the use of others to faciliate interaction, learn about emotions and generally mediate social interactions is just not a an easy thing to acomplish with an AS subject. The 'partner' that turns up today may not be there tomorrow. So how do you coach an AS teenager to fall back on there own resources?

The authors state that the book is suitable for use by parents, adolescents and adults, teachers and therapists (not many left out there). Personally I found this to be the most questionable claim of the whole book. How on Earth can it be a manual satisfying the requirements of such different audiences. It is verging on cyncical to suggest it has so much to offer to so many.

In conclusion, there are aspects of the book that are useful and other aspects that I found incongrous, if not downright peculiar. If I had a larger budget, I would definitely prefer Kathleen Quill's book, Do-Watch-Listen-Say even though it is not explicitly aimed at adolescents, and couple it with one of the Boystown Teaching Basic Social Skills to Youth as a more convincing pair. The 5-minute pediatric consult is written in an easy to read outline format. The writers have eliminated unnecesary obscure data and offer a concise outline of all major pediatric diseases. The topics are designed to be read in 5 minutes or less and all the up to date information to diagnose and treat a specific illness is included. The topics are alphabetized, so they are easy to look up. The writers are accomplised experts in their fields and the book has been edited by the distinguished Dr. Schwartz, at the Children's Hospital of Philadelphia. As a professor of Pediatrics, I highly recommend this book to practicing pediatricians, family practitioners, nurses and students.

Miscellaneous: Trisomy 21, epidermolysis bullosa, etc.

III. Preoperative Considerations

This chapter will discuss monitors, airway equipment, and induction & maintenance of general anesthesia.

V. Postoperative Concerns

This chapter will discuss routine care and monitoring of pediatric patients postoperatively. It will include controversies about discharge criteria, postoperative pain management, and common complications such as nausea/vomiting and stridor.

VI. Specialty Anesthesia

This chapter will include specifics about managing common pediatric surgeries. It will be organized into subspecialty areas that will include: ENT, Cardiac, Ophtho, Ortho, Thoracic, Abdominal, Neonatal emergencies, Dentistry, Urology, Neurosurgery, Transplants, Radiological procedures, Radiation therapy, Endoscopy, and Sedation for painful procedures.

VII. Critical Care

This chapter will introduce the resident to the essentials of pediatric critical care. Since the ABA does not require too much knowledge in this area, this chapter will not be very detailed, but will include principles of resuscitation (PALS & NALS).

VIII. Acute & Chronic Pain Management

This chapter will briefly review principles of acute and chronic pain management in children.

Case Studies

At the end of each chapter there will be one to three case studies that are related to the chapter content. These can be contained within the gray boxes, or the gray boxes can be reserved for more traditional lists, or tables. Each case study will consist of a case and several questions to be answered about the case. The answers can either be listed directly below the questions, or preferably, further down so the reader can formulate their own answer first. The case study will ideally describe a controversial issue.

Example of case study from the Postoperative Concerns chapter:

A 3-month-old child presents for inguinal hernia repair. The child was born at 25 weeks gestation, and was ventilated for the first three weeks of life. Ongoing medical problems include anemia, and chronic lung disease of infancy.

This book is an absolute "must have" for obstetrical practitioners and expectant parents who are looking for a way back into normal childbirth. Medicalized obstetrics has taught their students and society that "techno-birth" is not only logical but safe. This book proves that the way we have been delivering babies for the past 40 years is not the safest most competent care we can give. Hopefully, after reading this book, the practitioner will come away with a renewed sense of purpose in providing safe, logical, evidence-based care, and the expectant parent will take a firmer stand against allowing unneccessary and hazardous "routine" birth interventions. We are all the better for this book and the many years of sound, documented research it provides us. The next generation of babies will thank us!

I work in a Children's Hospital doing inpatient coding. I see the range from infections disease to neurology to neonatal intensive care. This book for me is a the best reference tool for pediatrics. I am able to look up a diagnosis or a disease pathway and get the necessary information in a concise manner. The tables in the book are helpful, too! I am glad I found this book in my daughter's college bookstore.

I didn't like this book. It was too big to read during the paediatrics course, but the index didn't contain even the most important topics, so it was impossible to use as a reference book.
I kept searching whole chapters to find the spesific subjects I was looking for. In the end I handed it back to the store and bought another book.
If you want to read Nelson's paediatrics, buy the original, big book, not this one.

When I used this book during my pediatric clerkship as a medical student, it proved to be a reliable source of excellent information, seemingly perfectly directed at what most attending ask for during the rounds and conferences. It gives a brief synopsis of pathophysiology, etiology, diagnostic criterion, management and prognosis of all but the rarest pediatric ailments. The well-organized approach makes this one of the ideal books for skimming before an exam, and yet the details provided also afforded much information for those who are preparing for a lecture.
Yet for pediatric residents (of which I am currently one), this book proves too superficial. I used it for a few of our monthly exams (when my internship labors gave me too little time to study our bible - NELSON), and I felt that it didn't quite deliver the necessary level. Not enough was mentioned about the syndromes that attendings love to ask about.
Overall, though, I love this book. It makes for a good "bathroom read" for both medical student and resident, but where the student can take it back to his desk, the resident should leave it in the bathroom magazine rack for future visits.

This text has the information you need, often in multiple chapters. However, the book is poorly laid out, with long chapters that are hard to get through. For example, in the growth and development chapter, you go from growth charts to theoretical psych in a matter of pages. This makes it difficult to cover material as it is presented in class/clerkship. Hopefully, Rudolph's Fundamentals is better. If not, it's Blueprints all the way!

I have been working as a Pediatrician in Pakistan since 1992. In Pakistan the Standard Text book Of Pediatrics is Nelson Text book of Pediatrics. But the most liked book is Nelson Essentials of Pediatrics. I have read all its previous edditons first as a trainee of Pediatrics and now as a working pediatrician. I think one should always keep with him this book and Harriet Lane Hand Book of pediatrics if one wants to remain current.

The O.T.s where I used to work recommended this book to me and it has been invaluable! It is enjoyable to read, with useful information that is easy to understand. I hesitated to buy it, initially, because of the price, the "line" drawings and the fact that it was written by O.T.'s (I thought I might find it too technical). It turned out to be well worth the cost and the illustrations work just fine. Anyone who needs to feed or work with moms feeding their babies should buy this book; it is incredibly useful and enlightening! It's a shame this book has such limited availability......

As prehospital professionals, we know several things about children; on average only 10% of our patients are pediatrics; we are aware of physiological differences based on age; we know how quickly they decompensate and pediatric calls make most of us very nervous. I read this book in conjunction with taking a 16 hour course on care for children by prehospital professionals; I found the information to be very helpful and know that my comfort level in the field when working with pediatric patients and their parents has been improved as a result.
